Dr Anastacia Tomson co-led the development of South Africa's first clinical guideline on gender-affirming hormone therapy. She is not learning as she goes. She helped write the guidelines that others follow, and she refined her LGBTQIA+ and trans-focused practice at Callen-Lorde Community Health Centre in New York City.

This is not a gatekeeping process. It is a clinical assessment to make sure we understand your full picture and can prescribe safely.
Before initiating or adjusting hormones, we establish a baseline through blood tests: hormonal levels, liver function, full blood count, and other relevant markers. The pathology itself is done off-site by an external provider (sometimes located in the same building, sometimes a private lab of your choice). They charge their own fees, separate from our consultation. Our doctors then interpret the results in the context of your goals and overall health.
We prescribe the appropriate hormones based on your goals, your baseline health, and the current evidence. We explain what to expect, when to expect it, and what side effects to watch for. No vague reassurances — specific, honest guidance.
Hormone therapy requires regular review. We see patients on a schedule appropriate to where they are in their journey, adjusting doses based on blood work and how you are feeling, not just a number on a page.
Hormones are part of the picture, not the whole of it. We attend to mental health, cardiovascular health, bone density, sexual health, and the other aspects of wellbeing that matter for trans patients — often in ways that other practices forget to address.
Yes. You do not need a referral from a specialist or a psychiatrist to book an appointment with us. We will do a clinical assessment at your first visit and discuss the appropriate path forward.
A full consultation — usually longer than a standard GP visit — covering your health history, your goals, and what hormone therapy would look like for you. We will also arrange baseline blood tests before any prescription is issued.
We are not gatekeepers. There is no enforced waiting period and no series of hoops to clear before treatment can begin. Your doctor advises a starting point based on your blood results and your readiness, following a thorough informed-consent process and clinical evaluation. For some patients that means starting at the second appointment, once baseline bloods are back; for others it means a little longer if there is something clinical worth understanding first. The timeline is yours and your doctor's to set together.
We are happy to take over ongoing care. Bring your existing prescription and any recent blood results, and we can continue your monitoring and management from there.
