MyFamilyGP is a private medical practice trading as Drs Tomson, Laskov & Associates (Practice No: PR 1407619), with rooms at Suite 404, 4th Floor, Sea Point Medical Centre, 11 Kloof Road, Sea Point, Cape Town. For privacy-related questions, contact info@myfamily.gp.

2. What information we collect.

To deliver clinical care, we collect and process the following categories of personal information:

• Identification information — name, ID or passport number, date of birth, gender (and pronouns if you share them with us)

• Health information — medical history, examination findings, test results, prescriptions, referral letters, correspondence with other healthcare providers, and notes about your care

We take particular care with information relating to identity, sexuality, gender, mental health, sexual health and any condition that has historically been used to discriminate against patients.

We will not share your personal or health information with anyone else without your consent or a lawful basis. Limited sharing happens with:

• Pathology laboratories that process your tests
• Pharmacies that dispense your prescriptions
• Specialists, hospitals, or other healthcare providers to whom we refer you
• Your medical aid, where you have given us permission to submit on your behalf
• Regulatory bodies where required by law
• Trusted IT service providers who help us operate our practice (e.g. our practice management software)

6. How long we keep your records.

Health records are retained in line with the requirements of the Health Professions Council of South Africa (HPCSA) and South African law — typically for at least six years after the last consultation for adult patients, and longer for minors and certain conditions.

7. Your rights.

Under POPIA, you have the right to:

• Request access to the personal information we hold about you
• Request correction of inaccurate or incomplete information
• Object to the processing of your information in certain circumstances
• Withdraw consent where processing is based on consent
• Lodge a complaint with the Information Regulator if you are not satisfied with how we have handled your information
